THIS MATTER came before the court upon Petition for Adoption Information filed by Michael Edward Chalek, alleging fraud on the court in the adoption proceeding of the instant case and requesting disclosure of his adoption records.
It is therefore
ADJUDGED that the Department of Children and Family Services is hereby directed to release to Mr. Michael Chalek a copy of the closed adoption record archived on microfilm. Disclosure of requested adoption information granted.
